I know that the requirements should be simple but for those who want to get a 100-percent approval rate, then bring all these documents on the day of your appointment:
The Basic Requirements are:
1. Birth Certificate, issued by Philippine Statistics Authority (PSA), formerly known as National Statistics Office (NSO), also known as the Census.
2. Marriage Certificate, issued by Philippine Statistics Authority (PSA), formerly known as National Statistics Office (NSO), also known as the Census. (If applicable only.)
3. One (1) Valid Identification Card, like:

SSS or GSIS UMID
Driver License
PRC ID
School ID or Alumni ID
Voters ID

4. NBI Clearance, old or newly issued, for local or travel abroad.
5. Other Supporting Documents, like:
If you're an employee, your BIR ITR Form 2316 or SSS E-1 Form
If you're a student, your Certificate of Registration, Registration Form or Enrolment Form
If you're a newly graduate, your Transcript of Records (plus your old school ID)
I know some people don't have the proper IDs or supporting documents mentioned above. But here's the truth, don't bother applying for a passport if you can't even get at least the ones mentioned above; you are just putting yourself in a discomfort.
Simple, but straightforward.
Reminders for Birth Certificates
Check your NSO/PSA Birth Certificates if they have all these Information, otherwise, go to your local civil registrar's office to have them fixed.
(C) First Name, (D) Middle Name, and (E) Last Name

(G) Month, (H) Day, (I) Year
DFA doesn't accept initial letters in the Middle Name. Some illegitimate children don't have middle names, and that's okay.
Some birth certificates are unreadable, so please get a local registrar certified true copy of the birth certificate as a supporting document.
Passport Fee
The passport fee is only 950 pesos if you're willing to wait 15 working days (not including Saturdays, Sundays and holidays).
You may also avail the 7 working days and pay 1200 pesos.

Renewal of Passport
There are 5 types of passport already issued by the DFA:
1. Brown Passport (with handwritten data, issued in the 1980s and below)
2. Green Passport
3. XX-Passport with Maroon Color (with passport no. starting from XX, issued between 2007 to 2010)
4. E-Passport with Maroon Color (has embedded chip, and passport no. starting with EA, EB, EC, issued starting 2010 to 2016)
5. E-Passport with Maroon Color (has embedded chip, new design, and passport no. starting with P, issued from 2016 to present)
Requirements for renewal of passport is to bring your most recent passport and photocopy the bio-page.
If you are renewing an old passport such as the (nos. 1-3) brown, green and xx-passports, requirements for first time application are still prescribed.
Minor Applicants

Minor applicants must be accompanied by parent, or by a consented representative. Their requirements are still that of the first time applicants, whether applying for the first time or renewal.
The accompanying parent must bring his or her original passport that is valid at least 6 months or more.
If both parents are abroad, get an adult representative and authorize them by sending them these two documents issued by the nearest Philippine Embassy/Consulate:
1. Special Power of Attorney
2. Affidavit of Support and Consent
If you cannot travel with your child, also ask your representative to secure a DSWD Travel Clearance for the minor.
